Brave Beginnings Care Package

If you live in Montana and recently learned that your baby/child has a complex heart defect that requires temporary relocation to a large hospital, please fill out the Brave Beginnings CHD Travel Care Package Request form form or email kalee@montanabravehearts.org. Anyone is able to request a care package, but we are only able to send 1 care package per family at this time.
